How to fade post-acne dark marks (PIH)

How to fade post-acne dark marks (PIH)

Post-inflammatory hyperpigmentation (PIH) is the brown, red, or purple mark left after a pimple heals. The dark mark itself isn't a scar; it's pigment that the inflammation triggered. The right combination of vitamin C, niacinamide, retinoid, and disciplined SPF can cut fading time roughly in half.

Why PIH happens

Higher levels of active melanocytes (the pigment-producing cells). When skin is inflamed β€” from a pimple, a scratch, even an aggressive product β€” melanocytes respond by producing extra melanin and depositing it in the surrounding skin. On lighter skin, the response is smaller and clears faster. On brown and black skin, more pigment is produced and it sits in the skin longer.

This isn't a flaw β€” it's the same melanin that protects melanin-rich skin from UV damage. It just makes PIH a bigger ongoing concern.

PIH vs. true scarring (important distinction)

  • PIH (dark marks): Flat. Brown, red, or purple. Heals on its own β€” just slowly. Topicals work.
  • True acne scarring: Textured β€” either raised (hypertrophic, keloid) or pitted (atrophic, ice-pick). Topicals help a little; in-clinic treatments are more effective.

The good news: 90% of what people call "scarring" is actually PIH β€” and that's treatable at home.

The fade-faster routine

Morning

  1. Gentle cleanse with oat cleansing gel.
  2. Vitamin C 5% serum on damp skin. In independent testing, 100% saw a reduction in dark marks.
  3. Niacinamide 10% + rice water serum on top (or layer the other order β€” doesn't matter much in practice).
  4. Moisturiser.
  5. SPF 30 minimum. Without this, the rest of the routine does almost nothing.

Evening (4–5 nights a week)

  1. Cleanse.
  2. Retinoid + squalane complex 1% moisturiser. The single best ingredient for fading PIH β€” 100% said skin was visibly more even after 2 weeks.

Evening (alternate nights)

  1. Cleanse.
  2. If still breaking out: salicylic acid 2% serum on active spots.
  3. Hydrating serum + moisturiser everywhere.

The SPF rant (because it matters this much)

UV light directly stimulates the melanocytes that make pigment. Every day you skip SPF on a fading dark mark, you essentially reset the clock. UV exposure can make a PIH mark take 12 months to fade instead of 4. There is no shortcut around this.

Find a daily SPF you genuinely don't mind wearing. Reapply on long-sun days. This is the single highest-leverage thing in the routine.

Realistic timelines

  • Fresh marks (1–3 months old): Visibly faded in 6–12 weeks with full routine.
  • Older marks (6+ months): 3–6 months of consistency before significant fade.
  • Very old marks (1+ year): Slower. Still possible with retinoid over 6–12 months.

The biggest variable is SPF compliance. Daily sunscreen users see roughly double the rate of fade.

What can make PIH worse

  • Picking pimples. Every pick triples the inflammation, triples the pigment response.
  • Aggressive scrubs. Physical exfoliation can trigger PIH on melanin-rich skin even without a pimple.
  • Skipping SPF. See above.
  • Irritating product layering. Over-using actives causes the kind of inflammation that produces more PIH.
  • Picking scabs. Let the patch do it.

FAQ

Why does my dark mark look red, not brown?

Newer marks (often called PIE β€” post-inflammatory erythema) are red because the blood vessels in the area are still dilated. They typically transition to brown over a few weeks before fading. Niacinamide and gentle care help both stages.

Will hydroquinone fade my marks faster?

Hydroquinone is a powerful pigment-suppressor but should only be used short-term and ideally under dermatologist guidance β€” misuse can cause a paradoxical darkening called ochronosis, which is more common on melanin-rich skin. The topical stack in this article is safer for ongoing use.

Can I use vitamin C and niacinamide together?

Yes. The old internet myth that they cancel each other has been thoroughly debunked. Use them in either order.

How long should I wait between layers?

30–60 seconds is fine. You don't need to time it precisely.

What about peels for dark marks?

In-office mandelic acid or low-strength glycolic peels can be effective for melanin-rich skin β€” done by a professional who knows how to dose down for darker skin types. Avoid DIY high-strength peels.
